Abstract Introduction: Targeted needs assessment which includes identifying the needs of learners is a key step of program development. However, this step is commonly underestimated in postgraduate medical education programs, including otolaryngology residency training. Determining the needs of otolaryngologists may help educators to design more purposeful continuing medical education training programs. Furthermore, needs of specialists may provide a clearer insight about effectiveness of the residency programs in that specialty. Objective: To determine training needs of otolaryngology specialists and to identify deficiencies in otolaryngology residency training programs. Methods: Seventy-eight otolaryngology specialists, who completed all data gathering forms properly, were included in this descriptive, cross-sectional study. Demographic data of the participants were collected. Training needs of the participants were determined in seven basic areas of otolaryngology via two-round Delphi method. The basic areas were otology-neurotology, rhinology, laryngology, head and neck surgery, pediatric otolaryngology, sleep disorders and facial plastic surgery. Additionally, we asked an open-ended question to investigate the reasons why the participants perceived themselves incompetent and undereducated, or why they needed further training in some of the basic otolaryngology areas. Results: Facial plastic surgery, otology-neurotology and head and neck surgery were the most cited training areas in the needs assessment. Training needs differed according to experience and place of work. Financial expectations, deficiencies in residency training, regression in knowledge and skills, and special interest were effective determinants on decisions of the participants while determining their training needs. Conclusion: Otolaryngologists need further training in some areas of their field due to different reasons. Determining these areas and reasons will help in designing more effective continuous medical education activities and residency training programs in otolaryngology.


Resumo: Introdução: A avaliação de necessidades específicas, que inclui a identificação das necessidades dos alunos, é um passo fundamental no desenvolvimento de programas educacionais. No entanto, essa etapa costuma ser subestimada em programas de pós-graduação em educação médica, inclusive na residência em otorrinolaringologia. Determinar as necessidades dos otorrinolaringologistas pode ajudar os educadores a projetar programas mais direcionados de treinamento em educação médica continuada. Além disso, a análise das necessidades dos especialistas pode oferecer uma visão clara sobre a eficácia dos programas de residência nessa especialidade. Objetivo: Determinar as necessidades de treinamento de especialistas em otorrinolaringologia e identificar deficiências nos programas de residência em otorrinolaringologia. Método: Este estudo descritivo e transversal incluiu 78 especialistas em otorrinolaringologia que preencheram todos os formulários de coleta de dados adequadamente. Os dados demográficos dos participantes foram coletados. As necessidades de treinamento dos participantes foram determinadas em sete áreas básicas da otorrinolaringologia com o método Delphi em duas rodadas. As áreas básicas foram otologia/neurotologia, rinologia, laringologia, cirurgia de cabeça e pescoço, otorrinolaringologia pediátrica, distúrbios do sono e cirurgia plástica facial. Além disso, uma pergunta aberta foi usada para investigar os motivos pelos quais os participantes consideravam ter pouco conhecimento ou ser incapazes de atuar nessa área, ou por que precisavam de mais treinamento em algumas dessas áreas básicas. Resultados: Na avaliação das necessidades, cirurgia plástica facial, otologia-neurotologia e cirurgia de cabeça e pescoço foram as áreas de treinamento mais relatadas. As necessidades de treinamento variaram de acordo com a experiência e o local de trabalho. Expectativas financeiras, deficiências no treinamento durante a residência, regressão do conhecimento e habilidades, além de interesse especial, foram determinantes efetivos nas decisões dos participantes ao identificar suas necessidades de treinamento. Conclusão: Por diferentes motivos, os otorrinolaringologistas precisam de mais treinamento em algumas áreas. A determinação dessas áreas e motivos ajudará a planejar atividades de educação médica continuada e programas de treinamento em residência em otorrinolaringologia mais eficazes.

Abstract: This study aimed to identify factors associated with the need for complete dentures in one dental arch or both, among the elderly population. The cross-sectional household study was conducted with a representative sample of elderly people (65 years or older) in the state of São Paulo, Brazil, in 2015. The dependent variable was the need for complete dentures (in one arch or both), and independent variables were socioeconomic and demographic conditions, social capital, self-perception of oral health and access to dental services. Simple and multinomial logistic regression models tested effect measures (p < 0.05). A total of 5,948 elderly people participated. Results indicated that those with greater chances of needing a complete denture in one arch were male (OR = 1.54; CI95%:1.04-2.29), with fewer household goods (OR = 2.25; CI95%:1.50-3.38), lower monthly household income: R$501-1500 (OR = 3.44; CI95%:1.27-9.35), R$1501-2500 (OR = 4.11; CI95%:1.50-11.27), R$2501-4500 (OR = 2.76; CI95%:1.10-6.95), self-reported need for a complete denture (OR = 4.75; CI95%:3.08-7.35), ≥3 years since last dental appointment (OR = 1.80; CI95%:1.06-3.05), and dissatisfaction with last dental appointment (OR = 1.80; CI95%:1.06-3.05). There were more chances of the need for complete dentures in both arches among older elders (OR = 1.44; CI95%:1.06-1.88), with lower monthly household income: R$ < 501 (OR = 4.45; CI95%:1.71-11.60), R$501-1500 (OR = 4.01; CI95%:2.14-7.51), R$1501-2500 (OR = 2.95; CI95%:1.64-5.32), < 3 years of education (OR = 1.45; CI95%:1.13-1.85), feeling unhappy (OR = 2.74; CI95%:1.35-5.57), self-reported need for a complete denture (OR = 8.48; CI95%:5.75-12.50), dissatisfaction with their mouth (OR = 2.38; CI95%:1.64-3.46), ≥3 years since last dental appointment (OR = 4.28; CI95%:2.85-6.43), and dissatisfaction with last dental appointment (OR = 4.28; CI95%:2.85-6.43). The several dimensions of the determinants of the need for a complete denture reflect the influence of both demographic and socioeconomic aspects, social capital, self-perception of oral health and access to dental services.

ABSTRACT OBJECTIVE To compare the access to and effective use of health services available among international migrants and Chileans. METHODS Secondary analysis of the National Socioeconomic Characterization Survey (CASEN - Caracterización Socioeconómica Nacional ), version 2017. Indicators of access to the health system (having health insurance) and effective use of health services (perceived need, appointment or coverage, barriers and need satisfaction) were described in immigrants and local population, self-reported. Gaps by immigrant status were estimated using logistic regressions, with complex samples. RESULTS Immigrants were 7.5 times more likely to have no health insurance than local residents. Immigrants presented less perceived need than local residents, together with a greater lack of appointments (OR: 1.7 95%CI: 1.2-2.5), coverage (OR: 2.7 95%CI: 2.0-3.7) and unsatisfied need. The difference between immigrants and locals was not statistically significant in barriers to health care access (α = 0.005). CONCLUSIONS Disadvantages persist regarding the access to and use of health services by immigrants as opposed to Chileans compared with information from previous years. It is necessary to reduce the gaps between immigrants and people born in Chile, especially in terms of health system access. This is the first barrier to effective use of services. The generation of concrete strategies and health policies that consider an approach of social participation of the immigrant community is suggested to bring the health system closer to this population.


RESUMEN OBJETIVO Comparar el acceso y uso efectivo de servicios de salud disponibles entre migrantes internacionales y chilenos. MÉTODOS Análisis secundario de la encuesta poblacional de Caracterización Socioeconómica Nacional (CASEN), versión 2017. Se describieron indicadores de acceso al sistema de salud (tener previsión de salud) y uso efectivo de servicios de salud (necesidad sentida, consulta o cobertura, barreras y satisfacción de la necesidad) en inmigrantes y locales, autorreportados. Las brechas por condición de inmigrante se estimaron utilizando regresiones logísticas, con muestras complejas. RESULTADOS Los inmigrantes presentaron 7,5 veces más chances de no tener previsión de salud que los locales. Los inmigrantes presentaron una menor necesidad sentida que los locales, en conjunto con una mayor falta de consulta (OR: 1,7 IC95%: 1,2-2,5), cobertura (OR: 2,7 IC95%: 2,0-3,7) e insatisfacción de necesidades. La diferencia entre inmigrantes y locales no fue estadísticamente significativa en barreras de acceso a atención en salud (α = 0,005). CONCLUSIONES Persisten las desventajas en acceso y uso a servicios de salud en inmigrantes en comparación con los nacidos en Chile en contraste con información de años anteriores. Es necesario reducir las brechas entre inmigrantes y nacidos en Chile, sobre todo en cuanto a pertenencia a un sistema de salud. Esta es la primera barrera para un uso efectivo de servicios. Se sugiere generar estrategias concretas y políticas en salud que consideren un enfoque de participación social de la comunidad inmigrante y, adicionalmente, acerquen al sistema de salud a esta población.

ABSTRACT

Objective: The study's goal has been to identify the needs of the relatives of people hospitalized in an Intensive Care Unit from the interior of the Paraná State. Methods: Data were collected from May to September 2017, through the application of the Critical Care Family Needs Inventory (CCFNI) to 55 patients' family members. It was performed a descriptive and inferential analysis. Results: The most valued needs were those related to information, safety in knowing that the family member is receiving the best treatment and having access/ possibility of being able to talk with the physician at the visit time. Conclusion: Knowing the needs of family members is an important tool to assess the aspects related to the delivered care, and also provides subsidies for implementing actions that guarantee better quality of care for the patients and their families


Objetivo: Identificar as necessidades dos familiares de pessoas internadas em uma Unidade de Terapia Intensiva (UTI) no interior do Estado do Paraná. Método: Os dados foram coletados no período de maio à setembro de 2017, por meio da aplicação do Inventário de Necessidades e Estressores de Familiares em Terapia Intensiva (INEFTI) a 55 familiares. Realizado análise descritiva e inferencial. Resultados: As necessidades mais valorizadas foram as relacionadas com informação, segurança em saber que o familiar está recebendo o melhor tratamento e de ter acesso/possibilidade de poder conversar com o médico no momento da visita. Conclusão: Conhecer as necessidades dos familiares se faz como uma importante ferramenta de avaliação dos aspectos relacionados ao cuidado prestado e fornece subsídios para implementação de ações que garantam melhor qualidade da assistência ao paciente e a família

Método: Los datos fueron recolectados en el período de mayo a septiembre de 2017, por medio de la aplicación del Inventario de Necesidades y Estresores de Familiares en Terapia Intensiva (INEFTI) a 55 familiares. Realizado análisis descriptivo e inferencial. Resultados: Las necesidades más valoradas fueron las relacionadas con información, seguridad en saber que el familiar está recibiendo el mejor tratamiento y de tener acceso/posibilidad de poder conversar con el médico en el momento de la visita. Conclusión: Conocer las necesidades de los familiares se hace como una importante herramienta de evaluación de los aspectos relacionados al cuidado prestado y proporciona subsidios para implementación de acciones que garanticen mejor calidad de la asistencia al paciente y la familia

ABSTRACT

Objetivos: analisar a associação entre a auto percepção e a necessidade clínica de tratamento ortodôntico e investigar a relação entre a necessidade de tratamento ortodôntico e o impacto nas atividades diárias comer, sorrir e falar, em adolescentes. Sujeitos e método: estudo transversal com 1.630 escolares com idade entre 15 e 19 anos, de 36 municípios do estado do Rio Grande do Sul. Os instrumentos utilizados para determinar a necessidade de tratamento ortodôntico foram o Índice de Estética Dental (DAI) e o Componente Estético (AC) do Índice de Necessidade de Tratamento Ortodôntico (IOTN). Os dados foram analisados com o programa SPSS 21.0, utilizando-se os testes Qui-Quadrado de Pearson ou Qui-Quadrado de tendência linear e a regressão de Poisson. O nível de significância adotado foi de 5%. Resultados: observou-se associação positiva entre o IOTN e o DAI. A proporção de sujeitos que perceberam a necessidade de tratamento ortodôntico aumentou com a recomendação feita pelo dentista. Entretanto, a maioria dos adolescentes entrevistados não percebia necessidade de tratamento ortodôntico. Após o ajuste, observou-se associação entre a maloclusão e o desempenho da atividade diária sorrir (RP = 2,03) e o sexo dos escolares (RP = 1,28). Também se observou associação entre o sexo dos adolescentes (RP = 1,23), a escolaridade do pai (RP = 1,21) e o impacto na atividade comer. A atividade diária de falar estava associada com a escolaridade do pai (RP = 2,11). Conclusão: existe associação entre a auto percepção de tratamento ortodôntico e a necessidade clínica. A maloclusão impacta a atividade sorrir entre os adolescentes, entretanto, o mesmo não ocorre com as atividades comer e falar. (AU)


Objective: to analyze the association between self- -perception and clinical need for orthodontic treatment, and to investigate the relationship between the need for orthodontic treatment and the impact on daily activities of eating, smiling, and speaking, in adolescents. Subjects and method: cross-sectional study with 1630 school children aged 15 through 19 years from 36 cities of the state of Rio Grande do Sul, Brazil. The instruments used to determine the need for orthodontic treatment were the Dental Aesthetic Index (DAI) and the Aesthetic Component (AC) of the Index of Orthodontic Treatment Need (IOTN). The data were analyzed with the SPSS 21.0 software using Pearson's chi-square or linear trend chi-square tests and Poisson regression. The significance level was 5%. Results: there was a positive association between IOTN and DAI. The proportion of subjects who perceived the need for orthodontic treatment increased with the recommendation from the dentist. However, most of the adolescents interviewed did not perceive the need for orthodontic treatment. After adjustment, there was an association of malocclusion with the performance of the daily activity of smiling (PR = 2.03) and the gender of school children (PR = 1.28). An association was also observed for the gender of adolescents (PR = 1.23) and paternal level of education (PR = 1.21) with the daily activity of eating. The daily activity of speaking was associated with the paternal level of education (PR = 2.11). Conclusion: there is an association between self-perception of orthodontic treatment and the clinical need. Malocclusion affects the activity of smiling among adolescents, but the same does not occur with the activities of eating and speaking. (AU)

Abstract Objective: The aim of the present study was to assess the oral health status and treatment needs of cardiovascular surgery patients. Second, the awareness of cardiovascular surgery patients regarding the association between oral health and heart disease was considered. Methods: Assessment of oral health status, oral hygiene practices and treatment needs of 106 hospitalized patients in preparation for cardiovascular surgery. Patients were interviewed using a structured questionnaire designed for this study and oral examination was carried out by a dentist. Results: The oral hygiene practices of the study cohort were not up to the standard. Patients' awareness of infective endocarditis was poor. Approximately 68% patients experienced dental caries as decayed teeth or missing teeth due to caries and filled teeth. The mean plaque index in the study group was 1.25. In this study cohort, the mean probing depth of periodontal pockets was 5.7±1.3, whereas the mean number of teeth with periodontal pockets > 6 mm was 0.5±0.9. A total of 84 (74.2%) of the patients required dental treatment. Conclusion: The principal finding in this study was that patients with heart disease had poor oral health. This study also highlights the importance of better interaction among all healthcare professionals to integrate oral health as part of comprehensive inpatient healthcare.

Abstract Objective: To translate, culturally adapt, and assess the internal consistency of the adapted version of the Needs of Parents Questionnaire (NPQ) to be used in Brazil with a sample of parents of hospitalized children. Method: Methodological study, essentially based on the DISABKIDS® method and conducted in a public university hospital. The stages included translation, assessment by an expert committee, back translation, semantic validation, and pilot study. Results: After the original instrument was translated into Brazilian Portuguese and then back-translated, the semantic validation was performed with 30 parents and adjustments were implemented until the translated version was understandable by most participants. Afterwards, a pilot test was implemented with another 59 parents, the internal consistency of which was satisfactory. Conclusions: The Brazilian version of the NPQ proved to be reliable and, after assessing other psychometric properties in the field study, it will be useful to assess the needs of parents of hospitalized children, qualifying nursing care.


Resumen Objetivo: Realizar traducción, adaptación cultural y evaluación de la consistencia interna de la versión adaptada del Needs of Parents Questionnaire (NPQ) en muestras con padres de niños hospitalizados en Brasil. Método: Estudio metodológico, fundamentado esencialmente en el método DISABKIDS®, desarrollado en hospital público universitario. Fueron realizadas etapas de traducción, evaluación por un comité de expertos, retrotraducción, validación semántica y estudio piloto. Resultados: Después del proceso de traducción y retrotraducción del instrumento original para el portugués, se produjo la validación semántica con 30 padres, realizándose adecuaciones hasta que la versión traducida fuera considerada comprensible por la mayoría. Después, fue realizado el estudio piloto con otros 59 padres, presentando valores satisfactorios de la consistencia interna. Conclusiones: El NPQ adaptado se demostró confiable y, después de la evaluación de propiedades psicométricas en el estudio de campo, podrá ser útil para evaluar las necesidades de padres de niños hospitalizados, cualificando el cuidado de enfermería.


Resumo Objetivo: Realizar a tradução, adaptação cultural e avaliação da consistência interna da versão adaptada do Needs of Parents Questionnaire (NPQ) em amostra de pais de crianças hospitalizadas no Brasil. Método: Estudo metodológico, fundamentado essencialmente no método DISABKIDS®, desenvolvido em hospital público universitário. Foram realizadas as etapas de tradução, avaliação por um comitê de especialistas, retrotradução, validação semântica e estudo-piloto. Resultados: Após os processos de tradução e retrotradução do instrumento original para o português do Brasil, procedeu-se à validação semântica com 30 pais, tendo sido realizadas adequações até que a versão traduzida fosse considerada compreensível pela maioria dos participantes. Na sequência, realizou-se estudo-piloto com outros 59 pais, cujos valores da consistência interna foram satisfatórios. Conclusões: O NPQ adaptado para o Brasil mostrou-se confiável e, após avaliação de outras propriedades psicométricas no estudo de campo, poderá ser útil para avaliar as necessidades de pais de crianças hospitalizadas, qualificando o cuidado de enfermagem.

Abstract: The objective of this study was to analyze the epidemiological profile of oral health of Sateré-Mawé indigenous people living in Barreirinha, Amazonas (AM), Brazil, and the Tikuna indigenous people living in the urban area of Manaus (AM), in addition to characterizing the need for endodontic treatment between the two ethnic groups. A total of 138 individuals participated in the study, of whom 98 were Tikuna and 40 were Sateré-Mawé; they were distributed in age groups ranging from seven to 75 years. A very high prevalence of caries was observed in both ethnic groups. For the Sateré-Mawé in the 7-12 age group, the decayed, missing, and filled teeth (DMFT) index presented a mean value of 3.17. Comparing the DMFT index and the need for endodontic treatment in each of the ethnicities, these variables were found to be correlated, because as the DMFT index increases, the chances of needing endodontic treatment increase. The Sateré-Mawé presented a higher prevalence of need for endodontic treatment compared to the Tikuna. The association of comorbidities and the need for endodontic treatment were demonstrated only in the Tikuna, and there was only a correlation of this necessity with the presence of diabetes mellitus (DM) in one case. The need to expand access to oral health in these communities is emphasized, taking into account geographical access and technological, environmental, linguistic, and cultural barriers.

ABSTRACT

ABSTRACT OBJECTIVE To estimate the proportion of unmet need for personal assistance for basic and instrumental activities of daily life and to evaluate socioeconomic inequalities related to the unmet need among community-dwelling Brazilian older adults. METHODS This was a cross-sectional study with data from the last National Health Survey in Brazil. Unmet need was considered as the presence of at least one unmet need for basic or instrumental activities of daily life among individuals reporting the need for assistance. Logistic regression models were used to assess the correlates and probabilities of unmet need. RESULTS The proportion of unmet need was 18% and 7.1% for basic and instrumental activities of daily life, respectively. Unmet need was significantly related to living arrangements and socioeconomic status. Individuals in the first quintile of wealth status had about 50% higher probability of having an unmet need. A family member was the most prevalent type of caregiver. CONCLUSIONS Long-term care policy is needed to reduce the proportion of unmet need, especially among socioeconomically disadvantaged groups. Future studies should address the availability, training, and remuneration of caregivers, as those are an indispensable labor force in an aging society.

ABSTRACT

Abstract The aim of this study is to investigate the relationship between the epidemiological and clinical profiles of patients before and after hematopoietic stem cell transplantation (HSCT) and the need for endodontic treatment. The subjects included 188 individuals enrolled in the dental care program for transplanted patients of the School of Dentistry, Federal University of Minas Gerais (Faculdade de Odontologia da Universidade Federal de Minas Gerais, FO-UFMG) from March 2011 through March 2016. The patients were subjected to an HSCT conditioning dental regimen based on a thorough clinical and radiographic evaluation. Intraoral periapical and bite-wing X-rays were obtained, and after evaluation, specific dental treatment was planned and performed. The following demographic and clinical data were collected from the patients' medical records: age, gender, transplantation stage, primary disease, transplant type, medication used, complete blood count at the time of visit, and need for endodontic treatment. The Kolmogorov-Smirnov and the chi-square tests were used. Leukemia (31.3%) and multiple myeloma (17.9%) were the most prevalent primary diseases. Most patients were subjected to allogeneic-related transplantation (83.6%). Most patients exhibited platelet counts and hemoglobin concentrations below the reference values in the pre-transplantation stage, while the neutrophil and platelet counts and the hemoglobin levels were within the reference ranges in the post-transplantation stage. The proportions of individuals requiring endodontic treatment were similar between the pre- and post-transplantation groups: 24.3% and 24.7%, respectively. The systemic conditions of the patients referred for dental treatment were compromised.

ABSTRACT

Estudo descritivo-exploratório conduzido junto a 411 pacientes e seus acompanhantes com o propósito de examinar a percepção e nível de satisfação de ambos quanto ao atendimento das necessidades de cuidados durante o período de hospitalização e, também, verificar a associação deste nível com variáveis sociodemográficas e características da hospitalização e da unidade. Na coleta de dados foi utilizado instrumento autoadministrado. O escore de satisfação dos pacientes/acompanhantes variou de 3,6(0,4) a 4,6(0,4). As necessidades emocionais e espirituais (81 e 82%), segurança (83 e 67%) e atenção (87 e 61%) foram as mais atendidas. Encontrou-se associação entre satisfação e algumas variáveis sociodemográficas e características da hospitalização. De maneira geral, os pacientes e seus acompanhantes mostraram-se satisfeitos com o atendimento das necessidades de cuidados, porém algumas áreas importantes apresentaram déficit de satisfação demandando maior atenção da equipe de enfermagem.


This is a descriptive-exploratory study conducted with 411 patients and their companions in order to examine their perception and level of satisfaction regarding the meeting of their care needs during hospitalization and to verify the association of this level with the socio-demographic variables and the characteristics of hospitalization and unit. A self-administered instrument was used for data collection. The patient/companion satisfaction score ranged from 3.6(0.4) to 4.6(0.4). The emotional and spiritual (81 and 82%), safety (83 and 67%), and care needs (87 and 61%) were the most met. An association was found between satisfaction and some socio-demographic variables and characteristics of hospitalization. Overall, patients and their companions were satisfied with their care; however, some important areas presented satisfaction deficits requiring more attention from the nursing team.

Alrededor del 75% de las personas morirá a causa de una o varias enfermedades crónicas progresivas. A partir de esta proyección, la OMS instó a los países a diseñar estrategias para el fortalecimiento de los cuidados paliativos como parte del tratamiento integral. En Cataluña, España, la medición de la prevalencia de estos pacientes según el debe decir NECPAL CCOMS-ICO© fue de 1.5% de la población. Es una evaluación cuali-cuantitativa, multifactorial indicativa y no dicotómica que completa el médico tratante. En Argentina no conocemos información sobre estos enfermos. Nuestro objetivo fue explorar y caracterizar en una población accesible de la Ciudad Autónoma de Buenos Aires la proporción de pacientes crónicos con necesidades de atención paliativa, mediante el NECPAL CCOMS-ICO©. Se relevaron los hospitales generales de la Región Sanitaria 2: (Piñero, Álvarez y Santojanni) y sus áreas programáticas. En la Región Sanitaria 1 se incorporó al hospital Udaondo de gastroenterología. Se entrevistaron 53 médicos (704 pacientes). Se identificó que 29.5% presentaba enfermedades crónicas avanzadas; el 72.1% de esos pacientes eran NECPAL positivos, más jóvenes que en otros estudios (mediana 64) y más del 98% tenían alto índice de comorbilidad. Se registró la demanda (31.4%) y necesidad (52.7%) de atención paliativa. Se describieron indicadores específicos de fragilidad, progresión y gravedad según enfermedad crónica. El principal hallazgo fue identificar, con un instrumento no basado en la mortalidad, que, en la ciudad de Buenos Aires, 1 de cada 3 pacientes con enfermedades crónicas podría morir en el próximo año y tenía necesidades de atención paliativa.


About 75% of population will die from one or more chronic progressive diseases. From this projection WHO urged countries to devise strategies for strengthening palliative treatment as part of comprehensive care. In Catalonia, Spain, direct measurement of the prevalence of these patients with NECPAL CCOMS-ICO© tool was 1.5% of the population. This tool is an indicative, not dichotomous, quali-quantitative multifactorial evaluation to be completed by the treating physician. In Argentina there is no information on these patients. Our goal was to explore and characterize the proportion of chronically ill patients in palliative care needs, by NECPAL CCOMS-ICO© tool, in an accessible population of the City of Buenos Aires. General hospitals of the Health Region 2 (Piñero, Álvarez and Santojanni) and its program areas were surveyed. In Health Region 1, we surveyed the Udaondo gastroenterology hospital. A total of 53 physicians (704 patients) were interviewed. It was identified that 29.5% of these patients were affected by advanced chronic diseases; 72.1% of them were NECPAL positive, younger (median 64) than in others studies, and more than 98% presented high levels of comorbidity. Palliative care demand (31.4%) and needs (52.7%) were recorded. Specific indicators of fragility, progression, severity and kind of chronic disease were described. The main finding was to identify, with an instrument not based on mortality that, in Buenos Aires City, 1 in 3 patients with chronic diseases could die in the next year and had palliative care needs.

A ausência do tema acesso e qualidade da atenção à saúde da pessoa com deficiência, no âmbito de todas as prioridades de pesquisa do Ministério da Saúde, ilustra a invisibilidade dessa população. Mais invisíveis ainda são as Pessoas com Deficiência Múltipla que necessitam de apoio pervasivo, eleitas como participantes deste estudo. Fundamentado nessa asserção, buscou-se analisar o acesso da Pessoa com Deficiência Múltipla que necessita de apoio pervasivo aos serviços de saúde bucal, à luz das seguintes dimensões: necessidade percebida, disponibilidade dos serviços, uso dos serviços de saúde, aceitabilidade, acessibilidade, adequação funcional e capacidade financeira. Para o alcance do objetivo, efetuou-se uma pesquisa de campo, descritiva, transversal, de âmbito regional, que se desenvolveu em Associações de Pais e Amigos do Excepcional do estado de São Paulo equipada com Unidade de Cuidados Diários ou unidade similar, nos municípios que compõem o Departamento Regional de Saúde VI - Bauru. O estudo contou com a participação de 71 pais e/ou responsáveis por Pessoas com Deficiência Múltipla que necessitam de apoio pervasivo, ou seja, constante, de alta intensidade, nos diversos ambientes. A investigação foi realizada por meio da aplicação de um questionário com perguntas abertas e fechadas aos pais e/ou responsáveis. Para a análise estatística foram utilizados os softwares: SPSS V17, Minitab 16 e Excel Office 2010. Os dados foram analisados por meio de três testes estatísticos - Teste de Igualdade de Duas Proporções; Intervalo de Confiança para a Média e P-valor. Os resultados, à luz das dimensões, denotam alta prevalência da percepção da necessidade de cuidados odontológicos pelos pais e/ou responsáveis por Pessoas com Deficiência Múltipla e presença de serviço de saúde bucal público acessível, no território. No entanto, a média de confiança dos pais na capacidade desses serviços em resolver prontamente a necessidade de saúde bucal da pessoa com deficiência múltipla foi expressa pelo número 1,42 ±0,58. Mais de setenta por cento dos pais e/ou responsáveis afirmaram que a Pessoa com Deficiência Múltipla não utiliza os serviços odontológicos de forma regular. A aceitabilidade relacionou-se diretamente com o perfil do profissional e com as características organizacionais dos serviços. A acessibilidade foi dificultada por obstáculos humanos, arquitetônicos (meios de transporte) e financeiros. A grande maioria dos pais verbalizou incapacidade de arcar com os custos de um tratamento odontológico, sem contar com a ajuda de alguém ou de algum benefício. Conclui- se que o acesso da Pessoa com Deficiência Múltipla que necessita de apoio pervasivo aos serviços de saúde bucal, que frequenta APAES com UCD, no DRS VI - Bauru, não é regular, nem oportuno e muito menos propicia o alcance dos melhores resultados possíveis para as especificidades da pessoa com deficiência. O acesso foi mediado, principalmente, por barreiras atitudinais e programáticas, ou seja, pelas atitudes, comportamentos e reações dos pais e/ou responsáveis pelas Pessoas com Deficiência Múltipla em relação ao cirurgião-dentista e as características dos serviços de saúde bucal, bem como das atitudes, comportamentos e reações dos cirurgiões-dentistas e dos serviços de saúde bucal em prestar assistência às pessoas com deficiência e aos seus responsáveis.(AU)


The absence of the theme access and quality of attention to health, concerning People with Disabilities, under all the research priorities of the Ministry of Health, illustrates the invisibility of this population. Still, more invisible are the People with Multiple Disabilities in need of pervasive support, elected as participants of this study. Based on this assertion, the access of People with Multiple Disabilities in need of pervasive support to oral health services was analysed in light of the following dimensions: perceived necessity, availability of services, use of health services, acceptability, accessibility, functional adequacy and financial status. To reach the goal, a regional, descriptive, cross-sectional field research was performed, which developed into Association of Parents and Friends of Disabled People of São Paulo state, equipped with Daily Care Unit or similar, in the municipalities taking part of the Regional Health Department VI - Bauru. The study had the participation of 71 parents and/or guardians of People with Multiple Disabilities in need of constant or high intensity pervasive support, in different environments. The research was conducted by applying a questionnaire with open and closed questions to parents and/or guardians. For statistical analysis SPSS V17 software, Minitab 16 and Excel Office 2010 were used. Data were analyzed using three statistical tests - Equal Test of Two Proportions; Mean Confidence Interval and P-value. The results, in light of the dimensions, denoted high prevalence of perceived need of dental care by parents and/or guardians for People with Disabilities and Multiple and presence of accessible public oral health services in the territory. However, parents' confidence concerning the ability of these services to promptly address the need for oral health of people with multiple disabilities was expressed by 1,42 ±0,58. More than seventy per cent of parents and/or guardians informed that the People with Multiple Disabilities do not use dental services regularly. Acceptability was related directly to the professional profile and the organizational characteristics of the services. Accessibility was hampered by human, architectural (means of transport) and financial obstacles. Most parents voiced inability to afford the cost of a dental treatment, unless they received help from someone or some benefits. It is concluded that People with Multiple Disabilities, in need of pervasive support for oral health services, attending APAES with UCD in DRS VI - Bauru, are not regular or timely, and do not achieve the best possible results for the specificities of disabled people. The access were mediated, primarily, by attitudinal and programmatic barriers, i.e., by attitudes, behaviors and reactions of parents and/or responsible for People with Multiple Disabilities, in relation to the dentist and to the characteristics of oral health services, as well as the attitudes, behaviors and reactions of the dentists and oral health services in assisting people with disabilities and their caregivers.(AU)

Abstract Sickle cell anemia (SCA) is the most prevalent genetic disease worldwide. Recurrent vaso-occlusive infarcts predispose SCA patients to infections, which are the primary causes of morbidly and mortality. This study aimed to evaluate the relationship between SCA and endodontic diseases. Personal information, medical data (hematological indices, virologic testing, blood transfusions, medications received, splenectomy) and information on the need for endodontic treatment were obtained from SCA patients who were registered and followed up by the Fundação Hemominas, Minas Gerais, Brazil.These data were compared with the need for root canal treatment in SCA patients. One hundred eight patients comprised the studied population, and the rate of the need for endodontic therapy was 10.2%. Among the medical data, a significant difference was observed for eosinophil (p = 0.045) counts and atypical lymphocyte counts (p = 0.036) when the groups (with and without the need for endodontic treatment) were compared. Statistical relevance was observed when comparing the patients with and without the need for root canal therapy concerned eosinophil counts and atypical lymphocyte counts. The differences in statistical medical data, observed between the groups suggest that both parameters are naturally connected to the stimulation of the immune system that can occur in the presence of root canal infections and that can be harmful to SCA individuals.

The aim of this study was to analyze the relationship between oral diseases and their impact on the daily performance of adult and elderly Brazilians, verify the association of oral diseases with socioeconomic and demographic features, and compare the standard estimate of need with the sociodental assessment of these same needs. The authors evaluated data from 17,398 Brazilians aged between 35-44 years and 65-74 years, taken from the cross-sectional Brazilian Oral Health Survey (Saúde Bucal Brasil - SBBrasil). Regression models were applied to assess associations among impacts on daily performance and income, schooling, gender, region, use of dental services, health perception and dental disease status. McNemar’s test was applied to compare standard versus impact-related estimates of need. The prevalence ratio of these impacts was associated with the sociodemographic versus health perceptions (p < 0.001) of adults and the elderly. Adults also had impacts associated with loss of periodontal attachment (p < 0.001). The prevalence of normative needs was 95.39% for adults and 99.76% for the elderly, whereas the impact-related estimate of need was 50.92% and 43.71%, respectively. The impacted-related approach had a statistically significant association with the normative estimate of need (p < 0.001). This study showed a relationship between oral impact on daily performance of adults and educational level. Sociodemographic features were also related to the impacts on both adults and the elderly, and to health perception. The impacts among the adults were related to the loss of periodontal attachment. In addition, the authors found a sizable difference between the standard versus the sociodental approach, in that the sociodental assessment needs were lower than the needs identified by the standard estimate of need.

OBJECTIVE: To determine the physical activity (PA) behavior, needs and preferences for underserved, ethnically diverse women with a history of endometrial cancer (EC). METHODS: Women with a history of EC (41 non-Hispanic black, 40 non-Hispanic white, and 18 Hispanic) completed a needs assessment during their regular follow-up appointments at Montefiore Medical Center in Bronx, NY, USA. An 8-week pilot PA intervention based on the results of the needs assessment was conducted with 5 EC survivors. RESULTS: Mean body mass index (BMI) among the 99 respondents was 34.1+/-7.6 kg/m2, and 66% did not exercise regularly. Self-described weight status was significantly lower than actual BMI category (p<0.001). Of the 86% who were interested in joining an exercise program, 95% were willing to attend at least once weekly. The primary motivations were improving health, losing weight, and feeling better physically. Despite the high interest in participation, volunteer rate was very low (8%). However, adherence to the 8-week pilot PA intervention was high (83%), and there were no adverse events. Body weight decreased in all pilot participants. CONCLUSION: These data show that ethnically diverse EC survivors have a great need for, and are highly interested in, PA interventions. However, greater care needs to be taken to assess and identify barriers to increase participation in such programs.

ABSTRACT

INTRODUCTION: The aim of this study was to identify factors associated with the prevalence of anterior open bite among five-year-old Brazilian children. METHODS: A cross-sectional study was undertaken using data from the National Survey of Oral Health (SB Brazil 2010). The outcome variable was anterior open bite classified as present or absent. The independent variables were classified by individual, sociodemographic and clinical factors. Data were analyzed through bivariate and multivariate analysis using SPSS statistical software (version 18.0) with a 95% level of significance. RESULTS: The prevalence of anterior open bite was 12.1%. Multivariate analysis showed that preschool children living in Southern Brazil had an increased chance of 1.8 more times of having anterior open bite (CI 95%: 1.16 - 3.02). Children identified with alterations in overjet had 14.6 times greater chances of having anterior open bite (CI 95%: 8.98 - 24.03). CONCLUSION: There was a significant association between anterior open bite and the region of Brazil where the children lived, the presence of altered overjet and the prevalence of posterior crossbite. .


INTRODUÇÃO: este estudo objetivou identificar os fatores associados à prevalência de mordida aberta anterior em crianças brasileiras com cinco anos de idade. MÉTODOS: foi realizado um estudo transversal analítico com dados do inquérito epidemiológico nacional de saúde bucal SB Brasil 2010. O desfecho estudado foi a mordida aberta, classificada em presente ou ausente. As variáveis independentes foram classificadas em individuais, sociodemográficas e clínicas. Os dados foram analisados por meio das análises bivariada e multivariada por meio do programa estatístico SPSS (versão 18.0), com nível de significância de 5%. RESULTADOS: a prevalência de mordida aberta anterior foi de 12,1% entre as crianças investigadas. Aqueles pré-escolares residentes na região Sul do Brasil apresentaram uma chance 1,8 vezes maior de serem diagnosticados com a mordida aberta anterior (IC 95%: 1,16 - 3,02). As crianças identificadas com alguma alteração de sobressaliência tiveram 14,6 vezes mais chance de pertencer ao grupo de crianças com mordida aberta (IC 95%: 8,98 - 24,03). CONCLUSÃO: verificou-se que mordida aberta anterior apresentou associação significativa com a região brasileira em que as crianças viviam, com a presença de alguma alteração de sobressaliência e com a prevalência de mordida cruzada posterior. .

OBJETIVO: Estimar a prevalência de cárie dentária e necessidades de tratamento em crianças de 12 anos e adolescentes. MÉTODOS: Estudo transversal com base nos resultados dos levantamentos epidemiológicos Condições de Saúde Bucal no Estado de São Paulo em 2002 e Pesquisa Nacional de Saúde Bucal (SBBrasil) 2010. Foram analisados os dados secundários de 5.782 crianças (2002) de 12 anos e outras 369 (2010); e para a faixa de 15 a 19 anos foram analisados 880 jovens (2002) e 300 jovens em 2010. A experiência de cárie foi avaliada pelo índice CPOD (dentes cariados, perdidos e obturados) e foram verificadas as necessidades de tratamento odontológico segundo os critérios propostos pela Organização Mundial da Saúde. O índice Significant Caries Index foi empregado para medir a severidade da cárie no terço do grupo que apresentou maior prevalência da doença. Para a análise dos resultados utilizaram-se os testes de Qui-quadrado e Mann-Whitney, com nível de 5% de significância. RESULTADOS: Houve diminuição de 39,3 pontos percentuais no índice CPOD aos 12 anos (p < 0,001) e de 41,1 pontos percentuais nos adolescentes (p < 0,001) entre 2002 e 2010, e aumento de aproximadamente 161,0 pontos percentuais e 303,0 pontos percentuais no grupo livres de cárie, respectivamente. A porcentagem de dentes restaurados diminuiu nos dois grupos etários, mas a prevalência de dentes cariados não se alterou para o grupo de alta experiência de cárie. No grupo de baixa experiência de cárie ocorreu diminuição do componente perdido para os adolescentes e aumento do componente cariado aos 12 anos e adolescentes. MÉTODOS: Estudio transversal con base en los resultados de las pesquisas epidemiológicas de las Condiciones de Salud Bucal en el Estado de Sao Paulo en 2002 y Proyecto SBBrasil2010. Se analizaron los datos secundarios de 5.782 niños (2002) de 12 años y 369 (2010); y para el grupo etario de 15 a 19 años se analizaron 880 jóvenes (2002) y 300 jóvenes en 2010. La experiencia de caries fue evaluada por el índice CPOD (dientes cariados, perdidos y obturados) y se verificaron las necesidades de tratamiento odontológico según los criterios propuestos por la Organización Mundial de la Salud. El índice Significant Caries Index fue empleado para medir la severidad de la caries en el tercio del grupo que presentó mayor prevalencia de la enfermedad. Para el análisis de los resultados se utilizaron las pruebas de Chi-cuadrado y Mann-Whitney, con nivel de 5% de significancia. RESULTADOS: Hubo disminución de 39,3 puntos porcentuales en el índice CPOD a los 12 años (p<0,001) y de 41,1 puntos porcentuales en los adolescentes (p<0,001) entre 2002 y 2010 y aumento de aproximadamente 161,0 puntos porcentuales y 303,0 puntos porcentuales en el grupo libres de caries, respectivamente. El porcentaje de dientes restaurados disminuyó en los dos grupos etarios, pero la prevalencia de dientes cariados no se alteró para el grupo con alta experiencia de caries. En el grupo con baja experiencia de caries ocurrió disminución del componente perdido para los adolescentes y aumento del componente cariado para los de 12 años y adolescentes. OBJECTIVE: To estimate the prevalence of dental caries and treatment needs in 12-year-olds and adolescents. METHODS: Cross-sectional study based on results from the epidemiological surveys: Oral Health Conditions in the State of Sao Paulo, 2002 and the Brazilian Oral Health Survey (SBBrasil) 2010. Secondary data for 5,782 (2002) and 369 (2010) 12-year-olds and 880 (2002) and 300 (2010) 15- to 19-year-olds were analyzed. Dental caries attack was evaluated using the DMFT (decayed, missing or filled teeth) index and the need for treatment verified using the criteria proposed by the World Health Organization. The Significant Caries Index was used to measure the severity of the decay in the tercile of the group with the highest prevalence of the disease. In order to analyze the results, the Chi-squared and Mann-Whitney tests were used, with a 5% significance. RESULTS: There was a decrease of 39.3 percentage points in the DMFT index for 12-year-olds (p < 0.001) and of 41.1 percentage points for the adolescents (p < 0.001) between 2002 and 2010, and an increase of around 161.0 and 303.0 percentage in the group which was free from dental caries respectively. The percentage of restored teeth decreased in both age groups, although the prevalence of dental caries did not change in the group more affected by dental caries. In the group with few dental caries, there was a decrease in the component ‘tooth loss’ for adolescents and increase in the component ‘decayed teeth’ for the 12-years-old and the adolescents. There was an increase in the need for dental treatment in the group as a whole and in the group of 12-year-olds more affected by dental caries; and among the adolescents, the need for restoration on two or more surface decreased in the group as a whole and also in the group which suffered least from dental caries. ABSTRACT

OBJECTIVE: To determine the normative orthodontic treatment need among 12-year-old Brazilian schoolchildren, in the municipality of Juiz de Fora, Minas Gerais, Brazil, and compare with the need as perceived by the children themselves and their parents or caregivers, assessing putative associated sociodemographic factors. METHODS: Four hundred and fifty one children without a previous history of orthodontic treatment were randomly selected from a population of 7,993 schoolchildren regularly attending the public and private educational sectors of the municipality of Juiz de Fora, Minas Gerais, Brazil. RESULTS: The prevalence of normative orthodontic treatment need in 12-year-old children, assessed with the Dental Aesthetic Index (DAI) was 65.6% (n = 155). The need perceived by the caregivers was 85.6%, and by the children was 83.8%. Only the perception by the caregivers maintained a significant correlation with the normative need of treatment when adjusted to the parents' schooling and economical level (p = 0.023). CONCLUSIONS: There is a high prevalence (65.6%) of malocclusion requiring orthodontic treatment in 12-year-old Brazilian schoolchildren. The most prevalent malocclusions in the study were: Crowding, Class II molar relationship and increased overjet. There was no significant correlation between the Index of Orthodontic Treatment Need - Aesthetic Component (IOTN-AC) related to dental aesthetic perception and the normative treatment need assessed with the DAI.


OBJETIVO: determinar a necessidade normativa de tratamento ortodôntico em escolares brasileiros de 12 anos de idade, no município de Juiz de Fora, Minas Gerais, e compará-la à necessidade percebida pelos responsáveis e crianças da amostra, avaliando potenciais fatores sociodemográficos associados. MÉTODOS: quatrocentos e cinquenta e uma crianças, sem história de tratamento ortodôntico, foram selecionadas, aleatoriamente, de uma população de 7.993 escolares matriculados na rede de ensino pública e particular da cidade de Juiz de Fora. RESULTADOS: a prevalência da necessidade normativa de tratamento ortodôntico em crianças de 12 anos de idade, utilizando o Índice de Estética Dentária (DAI), foi de 65,6% (n = 155). A percepção da necessidade pelos responsáveis foi de 85,6% e pelas crianças foi de 83,8%. No entanto, somente a percepção dos responsáveis teve uma correlação significativa com a necessidade normativa (p = 0,023). CONCLUSÕES: existe uma alta prevalência (65,6%) de má oclusão com necessidade de tratamento ortodôntico em escolares brasileiros de 12 anos de idade. As más oclusões mais prevalentes no estudo foram apinhamento, relação molar de Classe II e sobressaliência maxilar. Não houve uma correlação significativa entre a percepção da estética dentária por meio do IOTN-AC (Index of Orthodontic Treatment Need) e a necessidade de tratamento normativo avaliada por meio do DAI.

ABSTRACT

Objetivo: Avaliar a percepção em saúde oral e as necessidades de tratamento odontológico dos trabalhadores de uma empresa metalúrgica - montadora de máquinas agrícolas - da região de Piracicaba. Metodologia: O tipo de estudo foi transversal e a amostra aleatória simples por conveniência. Participaram deste estudo 156 trabalhadores que responderam a um questionário composto por 15 questões enfocando: identificação, percepção em saúde oral, satisfação com a saúde oral, hábitos de higiene oral, consultas ao dentista; e foram submetidos a exame clínico. A análise estatística compreendeu o uso do teste quiquadrado com nível de significância de 5%.Resultados: 86,5% dos trabalhadores eram do gênero masculino e que possuíam hábitos de higiene semelhantes em freqüência, independentemente do grau de escolaridade. Revelou que a maioria dos trabalhadores relatou problemas de saúde oral como cárie, doença periodontal e necessidade de prótese; e que os trabalhadores que possuem melhores hábitos de higiene e freqüentam o dentista com maior assiduidade não apresentam melhor percepção de sua condição oral. Com relação à condição oral, segundo os exames clínicos, maioria dos trabalhadores apresenta necessidade de pelo menos um dos problemas odontológicos investigados, sendo 60,9% em dentística,16,7% em cirurgia, 56,4% em periodontia, 56,2% em prótese e 14,2% em ortodontia. Conclusões: Parcela significativa dos trabalhadores é acometida por problemas orais amplamente conhecidos como a cárie, problemas periodontais e necessidade de prótese, sendo a percepção destes ineficientes para a detecção de tais problemas; o conhecimento acerca dos procedimentos de higiene oral é ineficiente para a manutenção da saúde oral dos trabalhadores; e há a necessidade da adoção de um programa em saúde oral de sensibilização e instrução aos trabalhadores.


Aim: To evaluate the perception in oral health and the dental care needs of workers from a metallurgical company – maker of farm machinery – the Piracicaba region, Brazil. Methodology: This is a cross-sectional study and the random sample of convenience. The sample comprised of 156 workers responded to a questionnaire composed by 15 questions focusing: identification, perception in oral health, satisfaction with oral health, oral hygiene, dentist’s appointments, and were submitted to a clinical examination. Statistical analysis was based on the chi-square, using a 5% significance level. Results: 86.5% of workers were male and that all of them had hygiene habits similar frequency, regardless of education level. Revealed that the majority of workers reported oral health problems such as caries, periodontal disease and need for prostheses, and that workers have better hygiene habits and attend the dentist more assiduously not have a better perception of their oral condition. Regarding the oral condition, according to the clinical examination, most workers need to present at least one of the investigated dental problems, being 60.9% in dentistry, 16.7% in surgery,56.4% in periodontics, 56.2 prosthetics and 14.2% in orthodontics. Conclusion: A significant proportion of workers affected by dental problems is widely known as caries, periodontal problems and need for prostheses, and the perception of ineffective for detecting such problems, the knowledge of oral hygiene procedures is inefficient to maintain the oral health of workers, and the necessity of adopting a program in oral health awareness and education workers.
